RESUMEN:El objetivo del presente trabajo fue caracterizar aspectos clínicos y epidemiológicos, así como la posible asociación con la muerte, del síndrome de diarrea neonatal (SDN) bovino, y determinar la frecuencia relativa de infección por Cryptosporidium spp. y E. coli F5+ en terneros con y sin diarrea neonatal de tambos comerciales de Uruguay. Se revisaron clínicamente y se colectaron muestras de suero sanguíneo y materia fecal de 556 terneros con (n: 271) y sin (n: 285) diarrea, de hasta un mes de vida, provenientes de 30 establecimientos lecheros de 7 departamentos. Se realizó un cuestionario a los productores o personal de los tambos, sobre datos generales y de manejo para detectar posibles factores de riesgo y/o protección. Las muestras de materia fecal se analizaron mediante un ELISA de captura comercial para detección de Cryptosporidium spp. y E. coli F5+. En 97 animales <8 días de vida, se estimó la concentración de proteínas séricas totales mediante refractometría. Cryptosporidium spp. fue detectado en 100% de los establecimientos (30/30), mientras que E. coli F5+ en 16.7% (5/30). Cryptosporidium spp. se asoció a la presentación de cuadros diarreicos [odds ratio (OR) 6, intervalo de confianza (IC) 95% 4.16-8.67, P <0.01] en la 2° semana de vida (OR 9.09, IC 95% 5.70-14.50, P <0.001). No se observaron asociaciones entre diarrea y muerte e infección por E. coli F5+. La diarrea se asoció positivamente con parámetros clínicos generales al momento del examen (actitud anormal, dificultad en la incorporación, reflejo de succión débil, onfalitis, elevada temperatura rectal), con variables predictoras del grado de deshidratación corporal (duración del pliegue cutáneo y tiempo de llenado capilar) y a la edad (2° semana de vida) de los terneros (OR 3.66, IC 95% 2.39-5.62, P <0.001). Los terneros diarreicos tuvieron más chances de morir que los no diarreicos (OR 1.71, IC 95% 0.89-3.31, P 0.10). Factores de manejo asociados con Cryptosporidium spp. fueron la rotación de la guachera (OR 1.42, IC 95% 0.93-2.16, P 0.098) y el suministro de leche de descarte (OR 1.33, IC 95% 0.94-1.87, P 0.10). Con respecto a la muerte de los animales, esta se asoció con varios de los factores de manejo, tales como la no exclusividad en las actividades laborales del operario de la crianza (OR 3.87, IC 95% 1.80-8.34, P 0.001), los sistemas de crianza colectivos (OR 4.60, IC 95% 0.47-5.02, P 0.006) y los establecimientos con mayor número de vacas (OR 27.1, IC 95% 6.42-114.70, P <0.001), entre otros. Los animales con una transferencia exitosa de la inmunidad pasiva tuvieron menos chances de sufrir diarrea (OR 0.38, IC 95% 0.16-0.91, P 0.03). Los resultados de este estudio permiten concluir que Cryptosporidium spp. está ampliamente distribuido en los establecimientos lecheros, que se asocia a SDN, y que ésta se asocia a su vez con muerte de terneros. El veterinario se puede valer del examen clínico para identificar y tratar precozmente a animales con diarrea en riesgo de morir. La prevención, con énfasis en el manejo, es fundamental para el control del SDN.
SUMMARY:The objective of the present study was to characterize clinical and epidemiological aspects, as well as the possible association with death, of bovine neonatal diarrhea syndrome (NDS), and to determine the relative frequency of infection with Cryptosporidium spp. and E. coli F5+ in calves with and without NDS from commercial dairy farms in Uruguay. Clinical examination was performed and blood serum and fecal samples were collected from 556, 1- to 30-day-old calves with (n: 271) and without (n: 285) diarrhea, from 30 dairy farms in 7 Uruguayan departments. A questionnaire was distributed among the 30 farmers and/or farm staff to gather information on general farm data and management practices, to assess possible risk/protection factors. Fecal samples were analyzed by a commercial capture ELISA for the detection of Cryptosporidium spp. and E. coli F5+. In 97 calves <8 days of life, total serum proteins were estimated by refractometry. Cryptosporidium spp. was detected in 100% of the farms (30/30), while E. coli F5+ was detected in 16.7% (5/30). Cryptosporidium spp. was associated with diarrhea [odds ratio (OR) 6, 95% confidence interval (CI) 4.16-8.67, P <0.01] in the 2nd week of life (OR 9.09, 95%CI 5.70-14.50, P <0.001). No associations were observed between diarrhea, death and E. coli F5+ infection. Diarrhea was positively associated with clinical parameters at the time of examination (abnormal attitude, difficulty standing up, weak suction reflex, omphalitis, elevated rectal temperature), and with variables predictive of the degree of body dehydration (skin pinch test, capillary refill time) and the age of the calves (2nd week of life: OR 3.66, 95% CI 2.39-5.62, P <0.001). Diarrheic calves were more likely to die than non-diarrheic calves (OR 1.71, 95% CI 0.89-3.31, P 0.10). Management factors associated with Cryptosporidium spp. were: rotation of the areas where the calves were housed (OR 1.42, 95% CI 0.93-2.16, P 0.098), and feeding the calves with waste milk (OR 1.33, 95% CI 0.94-1.87, P 0.10). Calf mortality was associated with several management factors, such as non-exclusivity in the work activities of the calf caregiver (OR 3.87, 95% CI 1.80-8.34, P 0.001), and larger size of milking cow herd in the farms (OR 27.1, 95% CI 6.42-114.70, P <0.001), among other factors. Animals with a successful transfer of passive immunity were less likely to suffer from diarrhea (OR 0.38, 95% CI 0.16-0.91, P 0.03). We conclude that Cryptosporidium spp. is widely distributed in dairy farms in Uruguay, the agent is associated with NDS, and this syndrome is in turn associated with calf mortality. Clinical examination performed by veterinarians allows for early identification and treatment of calves with NDS, which are crucial to reduce the risk for mortality. Prevention and management practices are key factors to control NDS.
